What does “cash-based physical therapy” mean?

Cash-based physical therapy means that this facility does not accept insurance. Essentially, this means that we are out of network with all insurance carries/companies & that we do not bill through insurance. Patients pay prior to each session, directly for each service that best suits their needs. This type of service allows our provider to focus on the patient, and not be directed by insurance.

Do I need a referral to receive treatment?

Based on Ohio’s current Physical Therapy Practice Act (Section 4755.481) an individual does not need a referral to be seen by a physical therapist. Ohio is a “Direct Access” state for physical therapy services, meaning an evaluation & treatment can be conducted without a prescription. Upon receiving patient’s consent, their physician will be notified within 5 days of their evaluation.

Per Dr. Costa’s preference, all post-op or surgical patient’s do require a surgical report &/or protocol from their surgeon.

Typically how long does a session last?

Depending which service is needed, time of sessions can vary. For our standard physical therapy services (evaluation & treatment), sessions typically last anywhere from 60-75 minutes. For those who are interested in a wellness screen or gait analysis, sessions typically last anywhere from 45-50 minutes.

Timing is subject to change, based on complexity of care. At Costa Rehab & Wellness, staff never wants the patient to feel like they are rushed.

What form of payments are accepted?

Cash, check, and credit/debit cards are accepted. Additionally, FSA & HSA accounts with either VISA or Mastercard are accepted as well.

Why choose a cash-based physical therapist?

There are many reasons that individuals choose a cash-based provider. Many private practices, typically treat multiple patients at once - limiting the 1-on-1 treatment sessions where a provider can really understand your needs and where the provider can get to know you not just as a patient, but as a person too. Cash-based practices often treat 1 patient at a time and focus on that sole patient for the entire duration of the session. This typically leads to quicker recovery outcomes with quality results.

What conditions do you treat? What ages do you treat?

Most conditions are treated at our facility, including the following categories of ailments: musculoskeletal, cardiovascular, neurological, pulmonary, and general overall wellness. Our facility typically treats individuals aged 9 years old & up. Any individuals under 9 years old, we often recommend to other facilities in the area that primarily treat pediatric patients, where they are better equip based on anatomy & physiology of the patient.

How soon can I get in to be seen by a provider?

At our facility, we make the best effort to get each patient evaluated or seen (especially in urgent cases), within 48-72 hours of the phone call.
